Overview: 
As a healthcare professional, you have likely heard the adage “if it is not documented, it was not done.” Documentation is a key factor in any malpractice lawsuit or licensing board matter. A well-documented record could be the difference between demonstrating your competency as a provider and making a credible case in your defense or losing the case. This session will discuss common documentation problems, provide documentation examples from actual cases, and suggest best practices in documentation and record-keeping to aid in the defense of malpractice lawsuits or licensing board investigations.


 

Objectives:  
  1. Identify common documentation and record-keeping deficiencies that expose nurses to liability.
  2. Describe elements that each healthcare record should contain.
  3. Develop processes and best practices related to chart review and record-keeping practices.
